Contract Services – Social Worker (East Kootenay Region) At Golden Life, you’ll be part of a trusted, values-driven organization that has been caring for seniors across Western Canada for over 25 years.
Golden Life Management is seeking to engage the contracted services of a Registered Social Worker (registered with the British Columbia College of Social Workers) to support our East Kootenay Villages, based out of Cranbrook, BC.
This role will primarily support:
Joseph Creek Village
Kootenay Street Village
The Social Worker will work collaboratively with site leadership and interdisciplinary teams to support residents and families across these communities.
Scope of Services The services will include, but are not limited to:
Conducting psychosocial assessments
Providing counselling services to meet the needs of residents and their families
Identifying and coordinating appropriate community resources
Advocating on behalf of residents
Providing education to staff, residents, and families related to:
Powers of Attorney
Substitute Decision Makers
Consent and capacity considerations
Public Guardian and Trustee processes
Maintaining accurate and timely resident records
Attending interdisciplinary case conferences as needed
Providing consultation to the Director of Clinical Care and Community Manager to support quality care
Supporting other related activities as mutually agreed upon
Contract hours are expected to be approximately 8 hours per week, with flexibility for additional hours in the future as operational needs evolve.
